By accepting this statement and placing an order to purchase Alerta Find, the buyer agrees to pay Track Patch 1 Corporation each month for twenty-four months and authorizes Track Patch 1 Corporation to charge buyer’s credit card each month for this amount. To cancel the recurring monthly amount before the 24-month period ends, buyer agrees to pay a $40.00 fee and must return product with a return material authorization number, which can be obtained by calling Track Patch 1 customer service at 1-833-325-3782. Monthly charges will stop during the billing period which follows receipt of returned product.

Alerta Patch Refill Kits

By accepting this statement and placing an order to purchase Alerta Patch Refill Kits, the buyer agrees to pay Track Patch 1 Corporation each month for twelve months and authorizes Track Patch 1 Corporation to charge buyer’s credit card each month for this amount. Alerta Patch Refill Kits will be delivered to customer per the frequency of the initial refill kit order (monthly, quarterly, semi-or annually). To cancel the recurring monthly amount before the 12-month period ends, buyer must call Track Patch 1 customer service at 1-833-325-3782. No refunds will be issued and product is not returned to Track Patch 1 Corp. Monthly charges will stop at the end of the current billing period.
